RE: Qualified Charitable Distribution from Individual Retirement Account 

Dear Sir or Madam: 

Please accept this letter as my request to make a direct charitable distribution from my Individual Retirement Account # _____________________________ to the Kalamazoo Bicycle Club (KBC). 

Please issue a check in the amount of $__________ payable to: Kalamazoo Bicycle Club (Registered Charity No. 38-2285331).

In your letter of transmittal to the KBC, please state my name and address as the donor of record in connection with this transfer. Please send a copy of your transmittal letter to me. 

And, please ask the KBC Treasurer to send a written acknowledgement of my contribution to me no later than January 31st of next year.

I intend to have this Qualified Charitable Distribution qualify for income tax exclusion during the current tax year.  Therefore, this distribution must be postmarked no later than December 31st.
